If you have identified that your business matches the eligibility criteria outlined on this website, and you have decided to apply, please use the online application form:

You will find clear instructions throughout the online form to help you complete your application, however if you require further information please refer to the FAQ page of this website for more details.
Please ensure you have the relevant information with you before you begin to fill in the application form. If you make an error in submitting the application form on line phone the INDEX Advice Centre 0121 204 3883 and we will help you.
TIPS FOR APPLYING ONLINE
Left click the mouse in the relevant textbox and type in the required information.
Repeat the above process to continue to the next text box or use the tab key.
Please note you can go back and change your responses in the same way.
In the voucher details section of the application form, left click the Innovation theme that best describes your business activity.
In the same way for the declaration by applicant section, left click to select the appropriate option button. If you make a mistake click another option button to change your response.
When all the relevant text boxes have been filled in click the submit button to send.
We would prefer all applications to be online, but if this is not possible for you please contact the INDEX Team to request a printed application form:
